Following completion of this web course, participants should be able to:
Identify prevalence of each area of need in US and WV
Identify ways to assess needs within practice settings (for home visitor team members and medical providers)
List what steps to take when a need has been identified
Identify the best possible approach for addressing each need
Recognize limitations for each approach within a rural setting
Discuss best suggestions for overcoming limitations
Implement best practices for advocating patients/clients based on specific challenges the individual is facing
Identify assessment strategies
Utilize templates for how to implement screening practices
Describe best practices for how to navigate patients/clients who have at least one need
